We want to ensure that your first appointment provides you with what you need to begin your pain management solution. On your initial visit you will be asked to fill in a questionnaire about your health in order to help us better plan your care. Please remember to complete the appropriate forms and bring them with you to your appointment so we can make the most of our time during your consultation.

Can I refer myself to your clinic?

Unfortunately you aren’t able to refer yourself. A medical doctor must refer all patients in order to be seen for Chronic Pain Management. Ask your doctor for a Chronic Pain Assessment today.

Do I have to have a family doctor before I can get a consultation for Chronic Pain Management?

All of our patients are required to have an active family physician that is willing to participate in your ongoing care and assume prescribing once you are discharged.

What should I bring to my first appointment?

It’s important that you bring all of your relevant medical information for your first visit with us. Please remember to bring your OHIP card as well as a list of any medications you are currently taking including over the counter medications. You will also be required to complete these forms and bring them with you along with any medical records you are able to provide including test results from other clinics.

How much does a visit cost?

Visits for Chronic Pain Management are covered under OHIP however some treatments (Botox, chiropractic and acupuncture) are not covered by OHIP.

What can I expect from my initial appointment?

Your first appointment with us is for Chronic Pain assessment only. If you are a candidate for our interventional pain management techniques, these options will be available to you and become part of your personal pain management plan.
